For Surgical Patients
We want to make sure that your healthcare experience starts off right. To help us expedite your check-in and registration, please be sure to bring the following items with you on the scheduled day of your procedure

Driver’s license or other valid identification card

Lab results, recent blood work, or any additional imaging reports from your doctor

Any medications you will need post-operatively, in their original pill bottles
4
Insurance cards

Payment for your insurance co-pay
Your physician should have provided specific information about dietary restrictions or other steps to take in the days leading up to your surgery. Please be sure to adhere to these closely to ensure that your procedure can proceed as scheduled.
